Privacy overview
We collect the name, contact information, preferred clinic/date and any details a patient voluntarily provides to process appointment requests. Admin users can access appointment records. Never send emergency medical information through this booking form.
Review moderation
Website reviews may be displayed publicly after consent and approval. Names are displayed as entered, and a verified badge is reserved for feedback whose patient status has actually been checked.
Contact and deletion requests
Contact the booking team at 03348511495 to request correction or deletion of information, subject to legitimate retention requirements. If email and Google Sheets notifications are enabled, booking contact details are also sent to the practice’s configured Google account to coordinate your visit. Optional clinical notes remain in the practice database.
